Pathway to Excellence Program Fees
Fees apply to all health care organizations including long term care facilities. Fees are subject to change.
Pathway Applicant Registration (P.A.R.) Application Fee - $3,750 (Includes Membership to the Pathway Learning Community)
Due at the time of the Pathway Applicant Registration (P.A.R.). Non-refundable.
Appraisal Process Fee
The invoice will be uploaded to the Online Application Portal one month before document submission date. Payment is due within 30 days of receipt.
Fees based on the number of licensed beds or visits per year.

|*Campus = For Pathway purposes, campuses are remote care sites of an in-patient organization (e.g., Hospital, Rehab, LTC facilities). The CNO of the applicant organization is accountable for nursing practice at the campus(es). The executive leadership and Board of Directors are shared across the applicant site and campus. Policies and procedures are the same for the applicant site and campus, as are ancillary resources. Campuses typically provide services to augment the full set of services offered at the primary site. If the additional campus is an acute care setting, the nurses are shared between the acute care sites. See more.||**Branch = For Pathway purposes, a branch is a remote care site of a free-standing ambulatory care organization (e.g., Clinics, ED, Urgent Care, Infusion Center, Medical Groups, Dialysis Center) that provides only ambulatory services. The CNO of the applicant organization is accountable for nursing practice at the branch(es). The executive leadership and Board of Directors are shared across the applicant site and branches. Policies and procedures are the same for the applicant site and branches, as are ancillary resources.|
Depending on availability, applicants requesting a Pathway Standards Document extension may choose from the subsequent 2 submission cycles. Applicants requesting to extend beyond 2 submission cycles must reapply.
Extension Fee: $1,000.00
Due if/when an organization requests an extension prior to 30 days of the approved document submission date.
Appraisal Preparation Cancellation Fee: $2,000.00
Due if/when an organization requests an extension within 30 days of the approved document submission date.
Document Submission Withdrawal Fee: $1,000.00
Due if/when an organization withdraws the application prior to 30 days of the approved document submission date.
Document Submission Withdrawal Fee: $2,000.00

Nurse Survey Translation Fee: $2,500.00
Due with Appraisal Process fee when translation of the Pathway Survey is required.
Interim Monitoring Reporting (IMR) Fee: $1,200.00 (only applicable to organizations that submitted their applications before January 2021)
One-Time Fee (4 Year Designation)
Effective February 1, 2019 Pathway to Excellence® organizations are required to submit an updated Organizational Demographic Form (ODF) and Contact Information form (CIF) electronically to the Pathway Program Office (PPO) on the second anniversary of their Pathway Standard Document submission. Organizations submit the initial ODF with their Pathway Standards Document (PSD) and the mid-cycle due date is two years from the PSD submission date. Data required is at the facility level, unit level data is not required. The ODF is submitted in Excel format file, not as a PDF file. A one-time fee is due with the mid-cycle ODF. The mid-cycle ODF and CIF are submitted by emailing firstname.lastname@example.org. If for any reason an organization is unable to submit the report within the required time frame, the organization must notify the PPO as early as possible. If the PPO does not receive the required documents by the last day of the organization's PSD submission month and the organization has failed to notify the PPO of a delay, a late fee of $500 will be charged. Failure to submit any required reports or failure to comply with Pathway requirements may be considered voluntary surrender of Pathway designation.
Documents must be submitted in English. Organizations outside of the US needing translation of the nurse survey must contact the Pathway Program Office immediately to discuss additional fees.
