Silver Spring, MD—The American Nurses Credentialing Center (ANCC) announced today that Southwestern Vermont Medical Center (SVMC), in Bennington, Vermont is the winner of the 2017 ANCC Magnet Prize™, sponsored by Cerner, a global leader in health care technology.
The prize recognizes innovative nursing programs and practices in ANCC Magnet®-recognized organizations. Cerner encourages cutting-edge nursing programs and practices by sponsoring the $50,000 purse attached to the ANCC Magnet Prize. The money will help SVMC further develop the innovative program that was selected. The award was presented today at the 2017 National Magnet Conference® in Houston.

SVMC was awarded the ANCC Magnet Prize for its development of an accountable community model for health. The project is rooted in the Mary Naylor, PhD, RN, FAAN Transitional Care Model, but has expanded to include the creation of an integrated care delivery system. The new model implements various innovations and allows for continuous evaluation of gaps and adjustments to delivery of care, specific to individual patient needs. The interdisciplinary team, led by nurses, strives to improve both the patient and caregiver experience, while decreasing costs and advancing the overall population health.
The model demonstrates the elements of the Magnet® Model while providing a comprehensive care plan for patients. Over the past three years, the project team has successfully applied its model to high-risk, chronically ill patients, behavioral health patients and patients dealing with substance abuse. Since the project’s inception, there has been a significant reduction in the number of emergency department visits and hospital admissions. Patients and caregivers alike have gained a better understanding of the patient’s ability to make decisions about their care and community collaboration has led to an improved knowledge base of resources and improved patient outcomes.

The Magnet Recognition Program® recognizes health care organizations that provide the very best in nursing care and uphold the tradition of professional nursing practice. Winning organizations must meet specific criteria such as extraordinary nursing activities that have been sustained over an extended period of time, innovations that are beyond the characteristics of Magnet organizations, and positive outcomes that have been demonstrated and empirically linked to the innovation.
