The U.S. Pharmocapeial Convention (USP) recently updated the Heparin
Sodium Injection and Heparin Flush Solution labels (including pre-filled heparin
flush solution). The total amount of heparin per vial is now highlighted on the label
instead of the typical amount of heparin per milliliter. The change in labeling has led
to dangerous heparin overdoses and fatal medication errors.
